To case manage young people leaving care and who have left care, including those with high and complex needs, who require personal advisor services.
To work jointly with the children looked after social worker to assess and plan for a young person whilst they are eligible and then taking on full case responsibility for a young people when they cease to be looked after.
To effectively communicate to young people what support they can except to receive via the leaving care service and their role in maintaining this support once they leave care.
To encourage and empower young people to participate in planning and decision making for their futures.
To promote positive outcomes for young people by supporting them to maintain accommodation, seek and maintain employment, education and/or training.
To support to the ongoing development of life skills and independent living for young people who have left care.
To assist young people to build resilience and reduce vulnerabilities (in areas such as domestic violence, drug and alcohol misuse offending behaviour, sexual exploitation).
To facilitate direct access to advice and information and universal and targeted support services that meet the specific needs of young people.
To ensure that young people are continually engaged with the identified support services.
To undertake evening visits and duties as and when required.
To provide advice, information and support to other professionals about the service.
To ensure that all electronic recording is completed in a timely manner and regularly updated.
To maintain the quality professional standards for the service.
To promote user participation and consultation in the design, delivery and evaluation of services to young people leaving care.
